I think that prefab is the one which is already made and is sold for assembly, as in off the shelf. On the other hand, custom made is the one which you make in house...

From what i know, i can only answer for you one part of that question. I too possess a Haas 5c indexer and it on an 11 pin controller therefore i would guess that the same case applies to your machine too.

All I know is that the MIC 6 Aluminum is more structurally sound, since it is usually stabilized thermally before it is released for application.
